Long Term Care (LTC Placement)

This program’s purpose is to ensure all consumers discharged from acute Psychiatric Hospitals and are in need of residentially based treatment, receive the most appropriate care to promote hope and  recovery. The program facilitates placement from hospitals to the most appropriate and least restrictive level of care for adults 18 to 59 years of age and coordinates with consumer and family, the regional clinic case manager, and the Public/Private Guardian. Other services include case management, financial benefits, research, and referral while the consumer is in a locked facility.
