Wraparound Program
Wraparound Services
As part of the Wraparound team, the PHN provides services for the youth in collaboration with Probation, Behavioral/Mental Health departments. The PHN works towards identifying the health-related needs of the clients and their families. Using a strengths-based approach, the PHN also provides community resources to clients as needed. The PHN provides the following services:
- Comprehensive health screening/assessment on all families receiving Wraparound services.
- Development of a specific plan for each family member based on assessment.
- Provision of linkages and referrals for health-related needs
- Provision of education, training, and consultation to the Wraparound team on health-related issues.
