You or your child’s doctor can make a referral to the CCS program. CCS will ask you to complete an application and provide financial information. CCS will also need medical reports from your child’s provider in order to determine eligibility.

You will be required to apply for Medi-Cal as part of the eligibility process. You may be required to pay fees to participate in the CCS program if you do not qualify for full scope Medi-Cal. The CCS Staff will let you know the fee amounts depending on your family’s income.

Once a child is determined eligible for CCS, a case is opened and CCS will authorize treatment and other services by CCS approved specialists to treat the eligible condition.

 

Your child may qualify if he/she:

  • is under 21 years old
  • has a health problem that is covered by CCS
  • is a resident of California
  • and has one of the following:
    • Medi-Cal, with full benefits
    • family income of $40,000 or less
    • out-of-pocket medical expenses expected to be more than 20 percent of family's adjusted gross income
    • a need for an evaluation to find out if there is a health problem covered by CCS
    • was adopted with a known health problem that is covered by CCS
    • a need for the Medical Therapy Program

Once the CCS office receives the complete referral and medical reports, the Intake Nurse will review them for medical eligibility determination. 

A CCS application packet will need to be completed by the client's parent/legal guardian or client over 18 years old to determine if the client is residentially and financially eligible for CCS Services. This can be done online by clicking " APPLY FOR CCS SERVICES "

Also, CCS will include a Notice of Privacy Practices so that the client and/or family know their rights and responsibilities under the Health Insurance Portability and Accountability Act (HIPAA). This Act is the federal law that establishes standards to protect the privacy and security of health information.

Once the CCS Program receives all of the information they need to make a determination, they will verify eligibility and either authorize the necessary service or deny the service. The client and/or responsible party will be notified by mail whether their request has been approved or denied. If an application or a service is denied, the client and/or responsible party will receive a Denial/Notice of Action (NOA) explaining the reason for the denial.
